{"id":7633,"date":"2021-07-12T11:17:36","date_gmt":"2021-07-12T09:17:36","guid":{"rendered":"https:\/\/agora.xtec.cat\/inssolderiu\/?p=7633"},"modified":"2021-10-24T08:42:37","modified_gmt":"2021-10-24T06:42:37","slug":"periode-matricula-ordinaria-cicles-formatius-grau-mitja","status":"publish","type":"post","link":"https:\/\/agora.xtec.cat\/inssolderiu\/general\/periode-matricula-ordinaria-cicles-formatius-grau-mitja\/","title":{"rendered":"Per\u00edode Matr\u00edcula Ordin\u00e0ria Cicles Formatius Grau Mitj\u00e0"},"content":{"rendered":"<p><b>PER\u00cdODE MATR\u00cdCULA ORDIN\u00c0RIA (CFGM Manteniment i Electromec\u00e0nica i V\u00eddeo, Discj\u00f2quei i So)<\/b><\/p>\n<p><b>DOCUMENTS NECESSARIS<\/b><span style=\"font-weight: 400;\">:\u00a0\u00a0\u00a0<\/span><span style=\"font-weight: 400;\">Nom\u00e9s aquells documents que no es van aportar en la preinscripci\u00f3.<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Full de matr\u00edcula <strong>(<\/strong><\/span><strong><a href=\"https:\/\/forms.gle\/hbuPSPsm7nmhoew76\" target=\"_blank\" rel=\"noopener noreferrer\">formulari<\/a>)<\/strong><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">2 fotografies de tamany carnet, amb el nom darrere (en el cas de no haver-se insertat en el formulari).<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Original i fotoc\u00f2pia del DNI, NIE, passaport o altres documents d\u2019identitat de l\u2019alumne\/a.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Original i fotoc\u00f2pia del Llibre de Fam\u00edlia.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Original i fotoc\u00f2pia de la TSI de l\u2019alumne\/a.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Original i fotoc\u00f2pia del DNI, NIE,\u00a0\u00a0passaport o altres documents d\u2019identitat del pare, la mare o el tutor\/a real.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">El\u00a0<\/span><b>carnet de vacunacions<\/b><span style=\"font-weight: 400;\">\u00a0on constin les dosis de vacunes rebudes i les dates corresponents\u00a0\u00a0(Alumnat ESO)<\/span><\/li>\n<\/ul>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Si no es t\u00e9 carnet de vacunacions cal presentar un certificat m\u00e8dic oficial o el carnet de salut amb les dosis de vacuna rebudes i les dates corresponents.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Si el nen o la nena no han estat vacunats per indicaci\u00f3 m\u00e8dica o per altres circumst\u00e0ncies, cal presentar un certificat m\u00e8dic oficial justificatiu.<\/span><\/li>\n<\/ul>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Certificat de conviv\u00e8ncia quan l\u2019adre\u00e7a del DNI no coincideix amb la de la sol\u00b7licitud.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">En el cas que la tutela nom\u00e9s la tingui un dels pares, c\u00f2pia de sent\u00e8ncia que aix\u00ed ho manifesti.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">En el cas d\u2019alumnes procedents de centres no adscrits, caldr\u00e0 que aportin una certificaci\u00f3 de les qualificacions obtingudes en aquest centre.<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">Alumnes preinscrits amb pla\u00e7a assignada (matr\u00edcula ordin\u00e0ria): del <\/span><b>12 al 16 de juliol de 2021<\/b><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<p><b>HORARIS PRESENTACI\u00d3 DOCUMENTACI\u00d3:<\/b><\/p>\n<p><span style=\"font-weight: 400;\">De dilluns a divendres de les 9:00h fins a les 13:00h.<\/span><\/p>\n<p><span style=\"font-weight: 400;\"><br \/>\n<\/span><b>INSTRUCCIONS \u00a0MATR\u00cdCULA<\/b><\/p>\n<p><b>QU\u00c8 S\u2019HA DE FER?<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Omplir el formulari i enviar-lo telem\u00e0ticament.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Imprimir el formulari (abans d\u2019enviar-lo), signar cada p\u00e0gina i portar-lo al centre juntament amb la documentaci\u00f3 sol\u00b7licitada.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Si no poden imprimir el formulari, el centre se\u2019n far\u00e0 c\u00e0rrec de fer-ho, per tal de poder signar aquest.
