Aix\u00f2 no obstant, trobem tres moments diferenciats:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Avaluaci\u00f3 continuada<\/b><span style=\"font-weight: 400;\">: es realitza mitjan\u00e7ant el seguiment i revisi\u00f3 de les diferents activitats planificades per determinar el seu grau d\u2019acompliment a partir dels seus corresponents criteris. Aquesta la realitza persona responsable, que s\u2019indica en cada fitxa de les activitats, en el moment de la finalitzaci\u00f3 d\u2019aquesta.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Avaluaci\u00f3 anual<\/b><span style=\"font-weight: 400;\">: a l\u2019inici de curs els objectius temporitzats per aquell per\u00edode s\u2019incorporaran la Programaci\u00f3 General Anual de Centre (PGAC). En finalitzar el per\u00edode escolar, l\u2019equip directiu mitjan\u00e7ant la Mem\u00f2ria Anual far\u00e0 el seguiment i recull de les evid\u00e8ncies de l\u2019assoliment dels objectius. Aquesta tasca es troba liderada per la comissi\u00f3 digital, que aprofitar\u00e0 per revisar els objectius establerts per al present i vinent curs, tot \u00a0 determinant la modificaci\u00f3, si fos necessari.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Avaluaci\u00f3 final<\/b><span style=\"font-weight: 400;\">: quan l\u2019EDC est\u00e0 arribant al final de la seva planificaci\u00f3, que s\u2019ha temporitzat a un termini de diversos cursos, es recullen totes les avaluacions anuals per tal\u00a0 de prendre una decisi\u00f3 en el si de l\u2019equip directiu. Actualitzar-la, per tant, plantejar-se una nova diagnosi i iniciar un nou pla o modificar l\u2019existent amb les dades que es disposen.\u00a0<\/span><\/li>\n<\/ul>\n<table>\n<thead>\n<tr>\n<th
