Si no es t\u00e9 el carnet de vacunacions, s\u2019ha d\u2019aportar un certificat m\u00e8dic oficial o el carnet de salut de l\u2019infant en el qual figurin les dosis de les vacunes rebudes amb les dates corresponents.
