Sempre s&#8217;haur\u00e0 d&#8217;adjuntar el passaport o i si \u00e9s estranger d&#8217;un pa\u00eds membre de la Uni\u00f3 Europea el document d&#8217;identitat del pa\u00eds d&#8217;origen.<\/span><\/span><\/li>\n<\/ul>\n<p><span style=\"font-family: Arial, serif;\">La documentaci\u00f3 que tots heu d\u2019aportar \u00e9s la seg\u00fcent:<\/span><\/p>\n<ul>\n<li><span style=\"font-family: Arial, serif;\">C\u00f2pia del <strong>carnet de vacunacions<\/strong> de l&#8217;alumne o l&#8217;alumna. Si no es t\u00e9 el carnet de vacunacions, s&#8217;ha d&#8217;aportar un certificat m\u00e8dic oficial o el carnet de salut de l&#8217;infant en el qual figurin les dosis de les vacunes rebudes amb les dates corresponents. Si el vostre fill o filla no ha estat vacunat per indicaci\u00f3 m\u00e8dica o per altres circumst\u00e0ncies, cal aportar un certificat m\u00e8dic oficial justificatiu.<\/span><\/li>\n<\/ul>\n<ul>\n<li><span style=\"font-family: Arial, serif;\">Fotoc\u00f2pia de la <strong>TSI<\/strong> (Targeta sanit\u00e0ria).<\/span><\/li>\n<li><span style=\"font-family: Arial, serif;\">1 Foto mida cartera i 2 fotos mida DNI si el vostre fill\/a \u00e9s per a Educaci\u00f3 Infantil 3 anys, <\/span><span style=\"font-family: Arial, serif;\"><b>amb el nom posat a darrera.<\/b><\/span><\/li>\n<li><span style=\"font-family: Arial, serif;\">2 fotos mida DNI si \u00e9s per a Prim\u00e0ria i per a ESO, <\/span><span style=\"font-family: Arial, serif;\"><b>amb el nom posat a darrera.<\/b><\/span><\/li>\n<li><span style=\"font-family: Arial, serif;\">Resguard del pagament corresponent de quota \u00fanica per curs de material
