Si no el teniu, sol\u00b7liciteu-lo al centre. Tamb\u00e9 el podeu consultar a:\u00a0<a href=\"http:\/\/educacio.gencat.cat\/ca\/tramits\/tramits-temes\/identificador-alumne\">http:\/\/educacio.gencat.cat\/ca\/tramits\/tramits-temes\/identificador-alumne<\/a><\/p>\n<p>Ompliu el formulari i abans d\u2019enviar-lo haureu d\u2019adjuntar escanejada o fotografiada la documentaci\u00f3 d\u2019identificaci\u00f3 i filiaci\u00f3 (DNI de qui fa la sol\u00b7licitud i de l\u2019alumne\/a i p\u00e0gines del llibre de fam\u00edlia on estan inscrits els pares i\u00a0 l\u2019alumne\/a).
