juny<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p>Per tal de formalitzar la preinscripci\u00f3 cal demanar hora d&#8217;entrevista per lliurar la seg\u00fcent documentaci\u00f3:<\/p>\n<ul>\n<li>La<a href=\"http:\/\/ensenyament.gencat.cat\/web\/shared\/OVT\/Departaments\/ENS\/TramitOVT\/preinscripcio\/obligatoris\/obligatoria_emp.pdf\" target=\"_blank\"> sol\u00b7licitud oficial de preinscripci\u00f3<\/a> que ser\u00e0 \u00fanica, i es presentar\u00e0 al centre demanat en primer lloc i la fotoc\u00f2pia de l&#8217;esmentada sol\u00b7licitud, un cop omplerta. \u00a0La presentaci\u00f3 de sol\u00b7licituds a m\u00e9s d&#8217;un centre o el frau i la falsedat en les dades comportar\u00e0 per al sol\u00b7licitant la invalidaci\u00f3 dels drets de prioritat.<\/li>\n<li>Original i fotoc\u00f2pia del DNI del pare i la mare o tutor\/a.<\/li>\n<li>Original i fotoc\u00f2pia de la targeta sanit\u00e0ria individual de l&#8217;alumne\/a (TSI)<\/li>\n<li>Certificaci\u00f3 municipal de conviv\u00e8ncia i resguard de la renovaci\u00f3 del DNI quan el domicili que s&#8217;al\u00b7lega no coincideix amb el DNI<\/li>\n<li>Original i fotoc\u00f2pia del document acreditatiu de qualsevol circumst\u00e0ncia que s&#8217;al\u00b7legui a efectes de barems.<\/li>\n<li><a
