Si l&#8217;alumne o alumna est\u00e0 en situaci\u00f3 d&#8217;acolliment, la resoluci\u00f3 d&#8217;acolliment del Departament de Treball, Afers Socials i Fam\u00edlies.<\/li>\n<li>Original i fotoc\u00f2pia del DNI del pare i la mare o tutor\/a.<\/li>\n<li>Original i fotoc\u00f2pia de la targeta sanit\u00e0ria individual de l&#8217;alumne\/a (TSI)<\/li>\n<li>Certificaci\u00f3 municipal de conviv\u00e8ncia i resguard de la renovaci\u00f3 del DNI quan el domicili que s&#8217;al\u00b7lega no coincideix amb el DNI<\/li>\n<li>Original i fotoc\u00f2pia del document acreditatiu de qualsevol circumst\u00e0ncia que s&#8217;al\u00b7legui a efectes de barems.<\/li>\n<li><a href=\"https:\/\/drive.google.com\/file\/d\/0B6UqktLXYhLzcGdPdmJqS1J2dHM\/view?usp=sharing\" target=\"_blank\"
